The unspoken concen behind every enquiry
Every conversation we have with a prospective client— or with the person making enquiries on their behalf — contains the same unspoken concern.
“If anyone finds out I am in treatment, it’s a huge problem.“
We have heard this said directly. We have heard it implied. We have worked with clients who carried this fear for years before finding us, and who spent those years managing a private crisis alone because the risk of being discovered seeking help felt greater than the cost of not seeking it.
This page exists to answer that question in full. Not with reassurances, but with the specific operational detail of how Istana’s confidentiality works — because for the people we serve, a promise is not enough. They need to understand the architecture.
No known address
The most fundamental aspect of Istana’s confidentiality model is one that surprises people when they first hear it: we do not own our residences. There is no Istana clinic, no known rehabilitation address, other than our office in Singapore there is no building that can be associated with us.
For each client, we identify, secure and prepare a private villa— in Bali, Barbados or Ibiza— exclusively for their stay. The villa is selected for its privacy, its security, and its suitability for the programme being delivered. It bears no connection to Istana. It carries no indication of its purpose. To the outside world, it is simply a private residence.
When your stay ends, that villa ceases to be Istana. The next client will be in a different property entirely. There is no address to be traced to, no location to be identified, no pattern that can be followed.
First names only
With the exception of senior management and your primary clinical team, no one at Istana will know your full name. Complementary therapists, domestic staff, drivers, security personnel — all know you by your first name only.
All internal communications use a standard nomenclature and a single identifying letter. If you came to Bali, internal references would be ‘IJ Bali [initial]’. Your full name does not appear in operational communications.
Your NDA or ours — or both
All Istana personnel are bound by confidentiality agreements before they join any client team. Before your arrival, every member of your dedicated team signs a confidentiality agreement specific to your engagement.
If you wish to supply your own NDA — whether through your legal team or your family office — we will sign it and return it before your arrival. Every person who will have contact with you will sign it. This is not an unusual request. We accommodate it routinely.
Operational discretion — the detail that matters
Confidentiality at the level Istana clients require is not achieved through policy. It is achieved through operational design. Some of the specifics:
All vehicles used during your stay are discreet and unbranded. Bookings for restaurants, activities and external appointments are made under the name of Istana personnel, not yours. If anything needs to be rented or purchased, it is done in the name of a team member. Nurses do not wear uniforms bearing Istana branding. No team member wears any identifying clothing.
Your support team receives only the information they need to serve you well. Before your arrival, they receive a brief that might read: ‘[Name], male, late 40s, high-functioning, interested in X, prefers not to discuss Y, has pressing business matters that may require flexibility.’ The clinical details of your presentation are held by your clinical team only.
Your clinical records
No clinical or medical records, test results, psychiatric assessments or psychotherapy notes are shared without your explicit consent. Your psychotherapist will not share session content with other team members without your permission — they may ask to share non-sensitive coordination information (such as ‘this client needs lighter scheduling for the next two days’), but only with your agreement.
On departure, you receive a full clinical report — medical, psychiatric and psychotherapeutic — which is yours to share with your home practitioners if you choose. You may request to review any record of sensitive information and ask that it be deleted within the confines of local law.
After your departure, all internal communications relating to your stay are deleted and shredded.
The people around you
We understand that your situation may involve other people — an executive assistant managing your schedule, a family office overseeing your affairs, a trusted adviser who has helped arrange your stay. We are experienced in working with these individuals and understand the particular discretion they require in their own right.
Our concierge and PA team can liaise directly with your EA or office, maintaining whatever cover story is appropriate and ensuring that your professional affairs continue without drawing attention to your absence. We have worked with security teams, board advisers, agents, and family offices. This is familiar territory.
What genuine confidentiality makes possible
The practical protocols above are not the point. They are the mechanism. The point is what becomes possible when someone who has been carrying a private crisis — alone, for months or years — finally arrives somewhere they can put it down.
When the mask drops — and at Istana, it always does, because there is no reason to maintain it, what remains is a human being. Often exhausted. Sometimes frightened. Always, in our experience, relieved.
The work of recovery begins there. Not in the clinical sessions, not in the treatment plan — but in the moment someone stops performing and allows themselves to be met exactly as they are.
That is what confidentiality makes possible. Not merely the absence of exposure, but the presence of safety. And in our experience, safety is where everything begins.
Frequently asked questions
Can I use my own NDA?
Yes. We welcome this. Supply it through your legal team or family office and we will arrange for every member of your dedicated team to sign it before your arrival.
Who knows my full name during my stay?
Only senior management and your primary clinical team. All other personnel — domestic staff, complementary therapists, drivers, support staff — know you by first name only.
What happens to my records after I leave?
All internal communications are deleted and shredded on departure. You receive a full clinical report, which is yours to use as you choose. You may request the deletion of any sensitive records within the confines of local law.
Can family members or my EA know where I am?
Yes, if you choose. We can arrange for trusted individuals to be briefed on your location and condition at whatever level of detail you decide is appropriate. We can also liaise directly with your EA or office on your behalf, maintaining whatever professional narrative is required.
Has Istana ever had a confidentiality breach?
No. Our operational model is designed to make such a breach structurally improbable rather than merely contractually prohibited. The combination of no fixed address, first names only, signed NDAs, deleted records and discreet operational protocols creates a system where confidentiality is maintained through design, not trust alone.
If you have questions about our confidentiality protocols or our approach to treatmentthat are not answered here, we welcome the conversation.
